Important notes from Unistel:
It is important that all samples are accompanied by the required order form and supporting documentation.

The first form (Cattle Test Request 2026) contains the full details of the Breeder / Owner. The second form (Details and Parentage Nomination Form 2026) is used for animal information and to indicate relationships if sire/dam/parentage verification is required, as well as any parent nominations and requested tests.
